To request official school documents (transcripts, enrollment verification, diploma replacement, I-20 replacement, etc.), please complete the Document Request Form and pay the required fee. Documents will not be released until payment is verified.
You can submit the form online in a few minutes, or download the PDF version to fill out by hand. Paper forms are also available at the Front Desk.
We do not accept fax or email requests.
